A letter explaining the option to pay an annual block fee for some services has been sent to all patients via regular mail in 2022. The links for payment are below, please select your physician to proceed.  We are continuing to offer the annual block fee.  You may choose to renew, or start the annual block fee at anytime.  The list of package options is below.  Choose which package is best for you.  If you received an invoice in the mail and wish to join the block fee for a year, please discuss with reception.
